7/10/2023 EnchantaVaughn Approved Behavior: At the start of session, the client was supported for displaying a good attitude and socializing safely with her family. Client verbalized positive statements in response to QMHP questions, indicating she was in a good mood. Client was able to stay focus during the session. Client was polite and courteous of her mother and sister as they worked and played around her in the home. Behavior: Client was able to transition into the next activity without conflict and proceeded to engage into meaningful conversation with QMHP. Client was attentive and focused and displayed a high level of confidence when reflecting on her feelings. Client shows positivity to authority by giving them the same respect they give her. Intervention: QMHP facilitated a family discussion to talk about common mistakes children with ADHD make and how she can make better choices. QMHP reviewed home expectations and how client should conduct herself around peers and authority. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement. QMHP processed client feelings and offered choices of what to do when faced with conflict or challenged in the home setting. QMHP inquired about habits and routine that encouraged client to be productive and attentive in her daily life functioning. QMHP encouraged client to make good choices and to utilize self-calming techniques (i.e. counting to 10 and deep breathing skills) and processed with her about her behaviors, the good and bad, as well as, individual expectations. Intervention: QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP reminded client of goals and objectives in order to motivate her to execute ISP. QMHP inquired about weekly activities, asking what she had planned and what she looked forward to that would bring her closer to her goals. QMHP collaborated with mom in regard to home rules and regulations, home management, effective teaching practices, following verbal and non-verbal instruction, role playing exercises, and an appropriate incentive system. QMHP educated client about using good manners at home and and how she can be rewarded for it. Response: Client expressed positive thoughts about herself and was willing to try her best to implement a healthy schedule to ensure she was on the right track. Client communicated effectively by exhibiting good eye contact and appropriate tone of voice. Client verbalized positive feelings about goals and objectives. Client communicated that she wants to work independently, so that she could gain clarity of her personal feelings without taking it out on someone she loves and care about. Client displayed good participation and was responsive to all instructions, interacting well with mom and QMHP. Client responded in a positive manner acknowledging the positive reinforcement that the QMHP provided. Client responded well to feedback and discussed things she can do better when it comes to her mental health. Response: Client understands that her treatment plan and how important it is to follow every day. Client was fully aware of her objectives and continued to do what she was told. Client displays leadership skills by helping others. Client acknowledged interventions and verbalized clear understanding of each intervention. Client reflected on her weekly plans and what new things transpired from being a part of the ADHD population. Client implemented the positive reinforcement provided by her counselor and was able to stay motivated and calm. As the client has made more of her own decisions, she reported increased satisfaction and function. Client agreed to behave and communicate a positive attitude when executing problem solving and self-control strategies. Client promised she would try her best at home everyday.
